I have the picture I want to use a an entry layout but I don't know how to make one so if someone would make one for me that would be greatly appreciated. Please comment if your willing.

You should check out the layout diaries here, there are some great ones. If none of those are what you want, then go to the Project Service, and learn to make your own.
